PAM Health is seeking a full-time Case Manager in San Antonio, TX to coordinate care for a designated patient population across the inpatient stay. You will review admission records to ensure appropriate utilization and determine admission level of care, while identifying discharge needs and coordinating a solid discharge plan.

Working with the interdisciplinary team, you will support financial and quality outcomes and maintain/payor relationships.

Formación

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• Three to five years of inpatient experience, preferably in acute, IRF, or LTACH settings.

Responsabilidades

• Coordinate care across the interdisciplinary team for a designated patient population.
• Review inpatient admission records to ensure appropriate utilization of hospital resources and admission level of care.
• Assess discharge needs and coordinate the discharge plan.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
