Job summary

Northeast Family Services is seeking a Case Manager in New Hampshire to coordinate in-home services, collaborate with the treatment team and families, and ensure timely documentation and reporting. Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field.
• Minimum 2 years' experience with children, youth and families.
• Valid driver's license and reliable vehicle.
• Authorized to work in the U.S. (no visa sponsorship).

Responsibilities

• Manage assigned caseload and ensure proper in-home service delivery.
• Collaborate with DCYF, treatment team, and families to create treatment plans.
• Document progress and complete required reports.
• Provide parent education and supervised visitation.
• Coordinate transport and ensure client safety.
