Job summary

Northeast Family Services in New Hampshire is seeking a Case Manager to serve as a liaison for the treatment team, client, families and collateral service providers.

The role involves managing caseloads, developing and documenting treatment plans with measurable goals, providing parent education and supervision, and coordinating with DCYF, courts and schools. A valid driver's license and the ability to work afternoons/evenings are required.

Qualifications

• Bachelor’s degree in social work, counseling, psychology or related field.
• Two years minimum experience working with children, youth and families in home or community settings preferred.
• Valid driver’s license with reliable and consistent vehicle access.
• Applicants must be authorized to work in the U.S.; we are unable to sponsor visas at this time.
• Access to high-speed, secure internet required.

Responsibilities

• Maintains assigned caseload and ensures proper in-home service delivery per DCYF referrals.
• Collaborates with DCYF to respond to referrals, create treatment plans and participate in meetings.
• Develops treatment plans with measurable goals in partnership with families and DCYF.
• Completes required documentation and reports for service delivery.
• Participates in reporting progress to the court system as requested (in person and in writing).
• Provides parent education and supervised visitation to children, adults and families.
• Offers parent coaching and support including parenting strategies, healthcare and educational needs.
• Interfaces with DCYF, NH courts, schools and other treatment team members to ensure quality service.
• Assesses client risk and consults with supervisor on safety concerns.
• Arranges transportation for clients as needed, including child car seat installation.
• On-call rotation to support families; flexible availability, including afternoons/evenings.
• Document all clinical contacts and maintain EHS records.
• Other duties as assigned.
