Job summary

Northeast Family Services in Vermont is seeking a Case Manager to coordinate in‑home and community‑based services for children, youth and families. You will liaise with the treatment team, DCYF and collateral providers to implement treatment plans and ensure safety and quality of care.

Responsibilities include caseload management, documentation, court reporting, parent coaching, and flexible evening hours. A bachelor's degree in a human services field and driving eligibility are required.

Qualifications

• Bachelor’s degree in social work, counseling, psychology or related human services field.
• Two years minimum experience working with children, youth and families in home or community settings preferred.
• Valid driver’s license with reliable vehicle.

Responsibilities

• Maintains caseload and ensures in‑home service delivery per DCYF referrals.
• Collaborates with DCYF to respond to referrals, create treatment plans, and document services.
• Develops treatment plans with measurable goals in partnership with families and DCYF.
• Completes required documentation of service delivery including assessments and reports.
• Reports progress to the court system as requested in person or in writing.
• Provides parent education and supervised visitation to children and families in home/community settings.
• Offers parent coaching on effective parenting strategies and meeting healthcare/educational needs.
• Ensures safety by enforcing supervised visitation expectations and assessing risk.
• Interfaces with DCYF, NH court system, schools and providers for quality service.
• On-call rotation to support families; flexible evening hours as needed.
• Documentation of clinical contacts through Electronic Health System.
• Other duties as assigned.
