Job summary

Metropolitan Center for Independent Living seeks compassionate case managers to support individuals with disabilities on CADI, TBI, and Elderly waivers. You will coordinate services, document communications, and help clients maintain independence in their communities.

Responsibilities include phone and email outreach to clients and partners, and occasional in-person visits twice a year (virtual visits possible). Training is provided on local resources to ensure client needs are met.

• Strong listening, organization, and documentation skills.
• Experience coordinating services for individuals to support independence.
