Nashville Rescue Mission is seeking a full-time Case Manager to join our Men’s Guest Services team. You’ll conduct intake assessments and develop short-term plans to guide guests toward exit and independence.

You’ll provide Christian counseling and advocate for guests with external agencies, always aligning with our faith-based mission. As a coach and case manager, you’ll coordinate with internal and external resources, monitor progress, and report outcomes.

Qualifications

• Bachelor’s degree or higher with 1 year relevant work experience; or Master’s degree; or 10 years relevant work experience.
• Must have committed personal relationship with Jesus Christ and belong to a Christian body.
• Must have a valid driver’s license and be able to drive Nashville Rescue Mission vehicles.

Responsabilités

• Conduct intake interviews assessing guest potential for NRM or other programs.
• Establish continuous contact with agencies and services providing assistance.
• Establish appropriate guest plans and covenant agreements.
• Refer guests to appropriate internal/external resources to achieve plan goals.
• Document progress and collect data in NRM database.
• Provide weekly updates on service utilization from entry to exit.
