Job summary

Responsibilities include weekly meetings with clients, connecting them to community resources, and documenting progress. Strong communication and motivational skills are essential.

Qualifications

• Bachelor’s degree in Social Work or a related field with experience in non-profit/human service.
• Knowledge of community resources and practices with high-risk populations.
• Ability to legally operate a motor vehicle and provide own transportation.

Responsibilities

• Provide holistic and comprehensive case management services including assessments and goal setting.
• Meet with clients weekly or bi-weekly, with emphasis on in-person meetings.
• Connect families with community resources and document referrals.
