Job summary

RiseMe is seeking a dedicated Case Manager at Will Rogers Head Start in Oklahoma City to provide comprehensive case management to individuals and families in need. You will assess client needs, develop individualized service plans, and coordinate housing, employment, healthcare, and educational resources to support self-sufficiency.

The role requires a Bachelor’s degree in social work, psychology, human services, or a related field, and a minimum of 2 years of experience in a community-service

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
• Minimum 2 years of case management experience in community service.
• Experience with diverse populations and low-income communities.

Responsibilities

• Assess client needs and develop individualized service plans.
• Coordinate access to housing, employment, healthcare, and education services.
• Monitor progress and adjust plans as needed to achieve outcomes.
• Maintain accurate and confidential case files and documentation.
• Advocate for clients within the community and with service providers.
• Collaborate with agency staff and community partners to improve service delivery.
• Provide crisis intervention and support as needed.
• Prepare regular reports on client progress and program outcomes.
