Job summary

Encompass Health is seeking a Case Manager for a PRN role to coordinate patient care from admission to post-discharge. You will work with an interdisciplinary team on treatment plans, discharge planning, and ensuring timely, quality care aligned with payor requirements.

The candidate should possess an active license in their discipline, a bachelor’s degree, and CCM/ACM certification or plan to obtain within two years.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of the applicable discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• Active license required for the discipline where licensure is mandated by the state.
• CCM® or ACM certification required OR must be obtained within two years of being placed in the Case Manager II position.
• Bachelor’s degree required for all other licensed/certified health care professionals; graduate degree preferred.

Responsibilities

• Coordinate with the interdisciplinary team to establish tentative discharge plans and contingency plans.
• Participate in planning for and execution of the patient discharge experience.
• Monitor patient experience across quality, timeliness, service appropriateness, and payor requirements.
• Facilitate weekly team conferences and coordinate treatment plan modifications.
• Complete case management addendums and required documentation.
• Review case management reports and plan appropriate actions.
• Identify trends from ACT meetings and collaborate to reduce ACTs.
• Meet with patients/families within 24 hours of admission and assess goals within 48 hours.
