Qualifications

• Bachelor's degree in social work or related field.
• Active Minnesota social work license.
• One year of case management experience with target populations.

Responsibilities

• Assess needs and strengths and develop CSSP with clients.
• Coordinate services and advocate for clients and families.
• Maintain CSP/ISP progress documentation and required timelines.
• Ensure providers are enrolled with Minnesota DHS and respond to reports.
