Job summary

Mount Olivet Rolling Acres – MORA – is a diverse nonprofit serving adults, seniors and individuals with developmental disabilities in the Twin Cities. We are seeking a Case Manager for Hennepin County, based in Chanhassen with a 40-hour M-F daytime schedule.

Starting salary is $59,000 annually, DOQ, with a comprehensive benefits package including health, dental, life, disability, retirement and generous paid time off.

Qualifications

• Bachelor’s degree in social work with an active Minnesota social work license or
• Bachelor’s degree in psychology, special education, nursing, sociology, or a closely related field.

Responsibilities

• Engage individuals and families to assess needs and risks.
• Identify providers and determine eligibility and level of service.
• Develop and implement a CSSP within county allocations.
• Coordinate direct services and advocate for clients.
• Assist clients in accessing services and resources.
• Document progress and ensure compliance with timelines.
