• At least 1 year of directly related case management experience is preferred or equivalent.
• Knowledge of community resources and service planning processes is expected.

Responsibilities

• Provide service coordination and develop individualized, person-centered plans for individuals with developmental and intellectual disabilities.
• Assist individuals and families in accessing needed services and supports, and maintain effective inter-agency relationships.
