Horizon Behavioral Health in Lynchburg, VA is seeking a Case Management Team Leader - DD to oversee contracted DD case management vendors, ensuring integration with behavioral health, primary care, dental care, and social connections for individuals served.

The role includes training vendors on internal systems including the Electronic Health Record, coordinating services, and supervising in-house DD Case Management staff when needed. The position is full-time and based in Lynchburg.

Qualifications

• Bachelor's degree in an applicable human services field is required; Master’s degree preferred in Social Work, Mental Health Counseling, or related health field.
• Minimum of 2 years of related clinical experience; supervisory experience preferred.

Responsibilities

• Oversee contracted DD Case Management vendors and provide oversight.
• Train DD Case Management vendors on internal and external systems, including the Electronic Health Record.
• Communicate the need for contracted vendors to ensure behavioral health, primary care, dental care, and social connections needs are met.
• When needed, begin providing DD Case Management and/or supervise in-house DD Case Management staff.
