Horizon Behavioral Health in Lynchburg, VA is seeking a Case Management Team Leader - DD to oversee contracted DD case management vendors, ensuring integration with behavioral health, primary care, dental care, and social connections for individuals served.

The role includes training vendors on internal systems including the Electronic Health Record, coordinating services, and supervising in-house DD Case Management staff when needed. The position is full-time and based in Lynchburg.

Qualifications

• Bachelor's degree in an applicable human services field is required; Master’s degree preferred in Social Work, Mental Health Counseling, or related health field.
• Minimum of 2 years of related clinical experience; supervisory experience preferred.

Responsibilities

Case Management Full Time Professional Lynchburg, VA, US

Salary Range: $46,349.00 To $67,206.00 Annually

Hours: full-time (37.5 hours per week)

Location: Lynchburg, VA 24501

Position Summary

The Case Management Team Leader - DD will collaborate and provide oversight to contracted vendors providing DD Case Management. The Team Leader - DD will be responsible for training the DD Case Management vendors on internal and external systems to include the Electronic Health Record. The Team Leader - DD will also communicate the need for contracted vendors to ensure that the behavioral health, primary care, dental care, and social connections needs of each individual receiving services are satisfactorily met. As the need for internal DD Case Management arises, and/or the need to add DD Case Management staff, this position will begin to provide DD Case Management, and/or supervise the in-house DD Case Management positions.

Required Education and Experience

• Bachelor's degree from an accredited college/university in an applicable human services field. Preferred: Master’s degree in Social Work, Mental Health Counseling, or related health field preferred.
• A minimum of 2 years of related clinical experience. Prefer prior supervisory experience in related field.

The definition, causes and program philosophy of developmental disabilities; Treatment modalities and intervention techniques, such as behavior management, independent living; skills, training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination. Different types of assessments and their uses in program planning; Individuals rights; Local service delivery systems, including support systems; Types of developmental disability programs and services; Negotiating with individuals and service providers; Observing, recording and reporting behaviors; Identifying and documenting an individual's need for resources, services and other assistance; Identifying services within the established service system to meet the individual's needs; Coordinating the provision of services by diverse public and private providers; Analyzing and planning for the service needs of developmentally disabled persons; Formulating, writing, and implementing individual specific service plans to promote goal attainment for recipients with developmental disabilities; Using assessment tools; Demonstrate a positive regard for individuals and families (e.g. allowing risk taking, avoiding stereotypes of developmentally disabled people, respecting individuals and families privacy; believing; To participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed; To drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to client @other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites; Willing and available to work modified schedules (weekends, holiday, etc.) as required.
