Job summary

Community Partners is seeking a dedicated Case Manager in Consolidated Services to support adults with developmental disabilities and/or brain injuries. You will be the primary liaison for individuals and families, ensuring seamless service delivery and caseload management.

• Minimum of two years’ experience in the human service industry preferred.
• Excellent oral and written communication skills.
• Exceptional attention to detail.

Responsibilities

• Primary contact for individuals and families, ensuring smooth service delivery for those on caseload.
• Oversee and manage programs with families and assist in supervising Family Managed Employees (FME’s).
• Collaborate with team to coordinate services and ensure quality outcomes.
