Job summary

Arbor Associates is seeking a dedicated Case Manager to support individuals with developmental disabilities and behavioral health needs in Westfield, Springfield, and surrounding communities. This is a full-time temporary role covering a leave of absence with a consistent Monday–Friday daytime schedule.

You will work directly with clients in their homes and in the community to promote independence, stability, and quality of life.

Assist with housing stability and household management
Coordinate and reinforce medical appointments and medication routines
Provide budgeting support and financial literacy coaching
Facilitate community integration and social skill development
Transport clients to appointments and community activities using your personal vehicle
Document progress and maintain case records in accordance with program requirements
