Join a Growing Behavioral Health Program
Social Clubhouse is expanding and seeking a compassionate and organized Case Manager to support adults living with mental health conditions through coordinated services, advocacy, and recovery-focused programming.
As our program continues to grow and expand services, this role offers an opportunity to join a supportive team and contribute to the development of a larger continuum of care for individuals in recovery.
This position combines case management, group facilitation, and care coordination within a structured partial care program that helps members build the skills needed for independent living.
For over 30 years, Social Clubhouse has provided structured day services for adults in recovery from mental health conditions. Our partial care program focuses on skill-building, community integration, and person-centered support.
Serve as the primary care coordinator for a caseload of program members
Conduct weekly individual sessions to support recovery goals
Develop and maintain person-centered service plans
Facilitate daily Home Group and end-of-day Wrap-Up sessions
Coordinate care with community providers, housing resources, and medical providers
Collaborate with Mental Health Specialists on therapeutic group programming
Monitor members’ housing, medical, and wellness needs
Maintain accurate documentation and service records
Training will be provided in IMR, Prevocational Services, and Co-Occurring programming.
You may qualify if you have:
Bachelor’s degree in social services, psychology, rehabilitation, or a related field
OR
Associate’s degree with at least two years of experience in human services
OR
Five years of experience working in human services or behavioral health
OR
Relevant professional credential such as CPRP, LAC, BSW, CRC, CADC, BSN, or related certification
