Job summary

Arbor Associates is seeking a dedicated Case Manager to support individuals with developmental disabilities and behavioral health needs in Westfield, Springfield, and surrounding communities. This is a full-time temporary role covering a leave of absence with a consistent Monday–Friday daytime schedule.

You will work directly with clients in their homes and in the community to promote independence, stability, and quality of life.

Responsibilities

• Support clients in developing and maintaining daily living skills
• Assist with housing stability and household management
• Coordinate and reinforce medical appointments and medication routines
• Provide budgeting support and financial literacy coaching
• Facilitate community integration and social skill development
• Transport clients to appointments and community activities using your personal vehicle
• Document progress and maintain case records in accordance with program requirements
