Job summary

One Hope United seeks a Mobile Crisis Case Manager to deliver community-based crisis intervention, case management, and mental health services for children and adults. Under supervision, you will stabilize crises, link clients to resources, and ensure documentation compliance.

The role includes in-home services, safety planning, and follow-up activities up to 90 days after crisis events. Strong organizational, communication, and confidentiality skills are essential.

Qualifications

  • BS/BA degree; BSW preferred in human service-related field.
  • 3+ years related experience in child welfare.
  • Valid driver’s license with acceptable driving record.

Job description

Mobile Crisis Case Manager

Under the assigned supervisor, the Mobile Crisis Case Manager provides community-based response, crisis intervention, case management, counseling, and mental health services to children and adults in accordance with agency policies and program guidelines. The Mobile Crisis Case Manager works to alleviate or stabilize mental health and behavioral health symptoms in referred clients that impair their ability to adjust to home, integrate into the community, or maintain living stability. Position provides linkages to resources that will help clients alleviate crisis. Crisis response services is provided to both adults and youth in the community. This position is full time.

Essential Duties and Responsibilities
  • Daytime crisis intervention services, hours starting at 8 am-5 pm.
  • Crisis calls may be extended past 5 pm, and staff are required to complete the crisis until coverage from the evening shift arrives
  • Some coverage of nights, weekends, and holidays, if schedule or staffing requires additional support
  • Conducts Crisis/Safety Assessment with an ETA of 90 minutes to respond in person.
  • Links clients to necessary resources i.e., health insurance, therapy, housing resources, medical treatment.
  • Provides in-home services to identify service needs, assess child and family safety, develop safety plans, and provide services to meet family and child needs.
  • Implements services under the guidelines of the practice framework, known as ARC (Attachment, Self-Regulation, and Competency)
  • Will provide follow-up services after crisis response, up to or beyond 90 days after the crisis. These services could include: follow-up through completion of Mental Health Assessments, Treatment Plans, and other Case Management services.
  • Provides mental health stabilization and specific services according to funding source (such as Medicaid) where applicable.
  • Completes required paperwork within established timelines and Medicaid-compliant documentation
  • Obtains and re-certifies for the IM CANS assessment tool yearly
  • Handles aspects of intake and assessment as assigned per program need.
  • Provides transportation for clients within program guidelines.
  • Assists in the integration of all services provided for the client by serving as a central point of communications for service components during crisis
  • Represents the agency at court hearings as necessary
  • Maintains comprehensive client files
  • Performs other related duties as assigned.

Qualifications

Education: BS/BA degree; BSW preferred in a human service-related field Experience: 3 or more years related experience in child welfare

Other: Valid driver’s license and acceptable driving record with OHU required levels of coverage for auto insurance
