Job summary

Duke Regional Hospital in Durham, North Carolina, seeks a skilled Case Manager to coordinate discharge planning and care transitions across the continuum of care. This role partners with interdisciplinary teams, patients, families, and community resources to optimize outcomes and resource use.

Ideal candidates hold a BSN or MSW, with at least 3 years of relevant experience and CCM/ACM/ANCC certification within 3 years of hire. RN license in NC is required for BSN candidates.

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant experience.
• Case Management Certification (ACM, CCM, or ANCC) required within 3 years of hire.
• For BSN-prepared candidates: Must hold a current RN license (or compact license) in the state of North Carolina.

Responsibilities

• Assess patients for case management needs: discharge planning, care transitions, education.
• Develop, implement, and evaluate individualized discharge plans.
• Coordinate services (Home Health, DME, transportation, SNF placement, rehab referrals).
• Monitor daily census and ensure timely assessments.
• Maintain communication with healthcare team and document findings.
• Provide education to patients and families on benefits and regulatory requirements.
