Job summary

Avita Community Partners is seeking a dedicated professional to oversee Crisis Residential Apartments (3 beds). You will perform case management, daily visits, and documentation in the electronic health record, while assisting with housing turnover and preparing units for new residents.

The role requires a Bachelor's degree in Social Work, Psychology, Mental Health, Counseling or related field and about 1 year of direct community mental health experience; 1 year in social services is preferred.

Qualifications

• Bachelor's degree in Social Work, Psychology, Mental Health, Counseling or related field.
• 1 year of experience in a community mental health setting providing direct services.
• Preferred: 1 year of full-time experience in social services delivery.

Responsibilities

• Oversee the Crisis Residential Apartments (3 beds) including daily visits and documentation in the EHR.
• Handle billing for services and support case management and skills development.
• Assist with housing turnover and prepare units for new residents.
