The role involves coordinating care, leading planning meetings, and ensuring timely assessments across service areas. A bachelor’s degree is required, with experience levels varying by Case Manager I-III.

Qualifications

• Bachelor's degree required.
• 1 year case management experience for Case Manager II; 2 years for Case Manager III.
• Graduate work may substitute for up to one year of experience.
• Strong writing and verbal skills; proficiency in MS Word/Outlook.

Responsibilities

• Maintain ethical, trauma-informed communications.
• Assist individuals with IDD in homes and community.
• Meet with caseload monthly to monitor services.
• Coordinate with Medicaid provider and state officials.
• Lead annual Person Directed Plan and joint meetings.
• Complete community first choice assessments and document state processes.
• Aid individuals in community living and independence.
• Respond to crisis situations.
• Possible travel across IDD service area.
• Adhere to agency Code of Conduct.
