CareStar, Inc. is seeking a Recovery Manager in Operations to conduct eligibility evaluations for specific programs and create person-centered service plans. The Recovery Manager will coordinate all required program services and assist individuals in accessing Medicaid, medical, social, educational, and other resources, regardless of funding source.

THE POSITION

CareStar is looking for motivated individuals to:

• Maintain knowledge of Medicaid, behavioral health systems, and community resources.
• Conduct face-to-face eligibility evaluations, including assessments, environment verification, and diagnosis confirmation, following program guidelines.
• Develop, monitor, and update person-centered care plans based on individual needs and preferences.
• Coordinate and authorize specialized recovery services.
• Assist individuals in connecting with necessary resources across funding sources.
• Adhere to CareStar's rules and policies, maintaining confidentiality and following acceptable use policies.
• Possess a Bachelor's Degree in social work, counseling, psychology, or a related field, or be a Licensed Social Worker (LSW).
• Have at least one year of post-degree experience working with individuals with chronic conditions or three years with individuals with severe mental illness.
• Have knowledge of issues affecting people with severe mental illness and community interventions.

This position is based in Licking County and covers surrounding areas.
