Recovery Manager – Guernsey and surrounding counties.

CareStar, Inc. is currently seeking a Recovery Manager in the Operations to conduct eligibility evaluations for individuals applying for specific programs and create a person-centered service plan. The Recovery Manager will coordinate all the program required services. He/She will assist the individual in gaining access to needed Medicaid services, as well as medical, social, educational, and other resources, regardless of funding source.

THE POSITION

CareStar is looking for motivated individuals who:

1. Maintain knowledge of Medicaid, the behavioral health system, and community resources.
2. Conduct face-to-face eligibility evaluations, including but not limited to, administration of the Adult Needs and Strengths Assessments; verification that the individual resides in a program approved environment; and verification of the individual's qualifying behavioral health diagnosis; other eligibility criteria as described in program specific guidelines ensuring that such evaluations are completed in accordance with legal, program, contract and/or CareStar requirements.
3. Develop, monitor, and update individual's person-centered care plans to ensure the individual's needs, preferences, health, and welfare.
4. Coordinate and authorize all specialized recovery services program services.
5. Assist individuals with connections to needed resources, including Medicaid services, medical, social, educational, and other resources, regardless of funding source.
6. Adhere to the CareStar Rule in performance of job responsibilities.
7. Understand and comply with CareStar Policies and Procedures.
8. Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
9. Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
10. Hold a Bachelor's Degree in social work, counseling, psychology, or a related field; or be a Licensed Social Worker (LSW).
11. Have a minimum of one-year post degree experience working with individuals with diagnosed chronic conditions and/or a minimum of three years’ post degree experience working with individuals with severe and persistent mental illness.
12. Possess knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals.

This position is open in Guernsey County and is responsible for covering the surrounding counties.
