Position Overview

Recovery Manager - Lucas and surrounding counties. This role involves conducting eligibility evaluations for individuals applying for specific programs, creating person-centered service plans, and coordinating required services. The manager will assist individuals in accessing Medicaid, medical, social, educational, and other resources, regardless of funding source.

Responsibilities

• Maintain knowledge of Medicaid, behavioral health systems, and community resources.
• Conduct face-to-face eligibility evaluations, including assessments and verification of diagnoses and residence.
• Develop, monitor, and update person-centered care plans.
• Coordinate and authorize recovery services.
• Assist individuals in connecting to needed resources.
• Follow CareStar policies, maintain confidentiality, and adhere to information system policies.

Qualifications

• Bachelor's Degree in social work, counseling, psychology, or related field; or Licensed Social Worker (LSW).
• Minimum one-year post-degree experience with individuals with chronic conditions or three years with severe mental illness.
• Knowledge of issues affecting those with severe mental illness and community interventions.

This position is based in Lucas County, covering surrounding counties.
