Recovery Manager – Noble and surrounding counties.

CareStar, Inc. is seeking a Recovery Manager in the Operations to conduct eligibility evaluations for individuals applying for specific programs and to create a person-centered service plan. The Recovery Manager will coordinate all required program services and assist individuals in gaining access to Medicaid, medical, social, educational, and other resources, regardless of funding source.

THE POSITION

CareStar is looking for motivated individuals who/to:

• Maintain knowledge of Medicaid, the behavioral health system, and community resources.
• Conduct face-to-face eligibility evaluations, including administering the Adult Needs and Strengths Assessments, verifying residence in an approved environment, and confirming behavioral health diagnoses, ensuring evaluations meet legal, program, contract, and CareStar requirements.
• Develop, monitor, and update person-centered care plans to meet individuals' needs, preferences, health, and welfare.
• Coordinate and authorize all specialized recovery services.
• Assist individuals in connecting with necessary resources, including Medicaid, medical, social, educational, and other services.
• Adhere to the CareStar Rule and comply with all policies and procedures.
• Maintain confidentiality of patient information and follow the Acceptable Use Policy for information systems.
• Possess a Bachelor's Degree in social work, counseling, psychology, or a related field, or be a Licensed Social Worker (LSW).
• Have at least one year of post-degree experience working with individuals with chronic conditions or three years with severe and persistent mental illness.
• Have knowledge of issues affecting people with severe mental illness and community-based interventions/resources.
