Position Overview

Community-Based Case Manager - Miami and Surrounding Counties

Are you motivated to make a difference in your community? CareStar, Inc. is seeking a Community-Based Case Manager in the Clinical Department to support individuals in maintaining independence and dignity by coordinating appropriate services. This role involves assisting participants in accessing services, developing care plans, and promoting self-determination.

Responsibilities

• Develop and implement care plans, including authorization of services and resources.
• Conduct home visits and monitor service delivery in compliance with program guidelines.
• Perform Person-Centered Planning, assessments, and annual Level of Care reviews, ensuring cultural considerations are integrated.
• Complete incident reports, identify high-risk situations, and take appropriate actions.
• Maintain confidentiality and adhere to organizational policies and procedures.

Qualifications

• Active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse.
• At least 12 months of experience in home and community-based services within the past 5 years.
• Knowledge of Medicare, Medicaid, Ohio Home Care waiver programs, and related regulations is desirable.

Location

This position is based in Miami County, covering surrounding counties.
