Community-Based Case Manager – Montgomery and Surrounding Counties

We are seeking a Community-Based Case Manager in the Clinical Department to assist individuals in remaining in the least restrictive environment while maintaining independence and dignity. The role involves coordinating services to ensure individuals receive appropriate, planned, and effective care, supporting their preferences and self-determination.

The Position

CareStar seeks motivated individuals who:

  • Hold an active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse.
  • Have at least 12 months of experience in home and community-based services within the past 5 years.
  • Possess knowledge of Medicare, Medicaid, home health care, and Ohio Home Care waiver programs (desirable).
  • Approve funds for service utilization, develop annual budgets, and meet program qualifications.
  • Stay current through continuing education, understanding Medicare, Medicaid, insurance, waivers, and ADA regulations.
  • Lead the development and implementation of consumer care plans, including service authorization.
  • Conduct home visits, monitor services, and ensure compliance with program expectations.
  • Perform Person-Centered Planning, assessments, and annual Level of Care reviews, considering cultural factors.
  • Complete incident reports, report high-risk situations, and take appropriate mitigation actions.
  • Adhere to CareStar's policies, procedures, and confidentiality standards.
  • Use information systems responsibly, following the Acceptable Use Policy.
