Community-Based Case Manager – Miami and Surrounding Counties

We are seeking a Community-Based Case Manager in the Clinical Department to support individuals in maintaining independence and human dignity by assisting with access to and coordination of services. The role involves promoting participant preferences, values, and self-determination.

THE POSITION

CareStar is looking for motivated individuals who:

• Hold an active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse.
• Have 12 months of experience in home and community-based services within the past 5 years.
• Possess knowledge of Medicare, Medicaid, home health care, and Ohio Home Care waiver programs (desirable).
• Approve funds for service utilization, develop annual cost budgets, and meet program qualifications and standards.
• Stay current through continuing education and utilize knowledge of Medicare, Medicaid, insurance, waivers, and relevant laws such as the Americans with Disabilities Act.
• Lead the development and implementation of the consumer’s care plan, including authorization of services.
• Complete home visits, monitor services, and ensure compliance with CareStar standards and program rules.
• Conduct Person-Centered Planning, health and safety assessments, social and environmental evaluations, and annual Level of Care assessments, addressing cultural considerations.
• Complete incident reports, report high-risk situations, and take appropriate actions to mitigate health risks.
• Adhere to CareStar policies, maintain confidentiality, and follow acceptable use policies for information systems.
