Job summary

Qualifications

• Minimum two years of related experience in case management or care coordination.
• Bilingual preferred.
• Valid driver's license and ability to obtain WestCare vehicle insurance.
• Familiarity with Broward County.

Responsibilities

• Conduct Needs Assessment
• Develop Service Plans with consumer and family
• Conduct monthly Service Plan reviews
• Document clinical services per licensing, Medicaid and Joint Commission
• Conduct individual case management sessions as needed
• Arrange transportation for consumers as needed
• Prepare monthly Progress Reports for courts and caseworkers
• Coordinate care with treatment team and external providers
• Develop discharge plan with treatment team
