Job summary

Macomb County is seeking a Case Manager for Children’s Severe Emotionally Disturbed (SED) to provide direct services, assessments, and care coordination within MCCMH. The role emphasizes person-centered planning, collaboration with families, and linkage to healthcare and community resources.

The position requires a bachelor’s degree in a mental health-related field and relevant experience; master’s degree preferred.

Qualifications

• Bachelor’s degree in psychology, social work, counseling or related field.
• Experience in case management or behavioral health preferred.
• Knowledge of community resources and care coordination.

Responsibilities

• Provides direct service to clients and families.
• Performs assessments and develops plans of service.
• Coordinates with health providers and aftercare services.
• Maintains documentation in EMR and participates in trainings.
• Monitors outcomes and reports to supervisor.
