Job summary

Responsibilities

• Provide services to children, adolescents and young adults with developmental disabilities and their families.
• Coordinate medical, psychiatric, social, educational, vocational, and residential needs.
• Engage in home visits, attend school meetings, courts and FAPT meetings.
• Maintain organized, self-driven schedule and adapt to changing circumstances.
