Job summary

Benchmark Human Services is seeking a dedicated Case Manager to support individuals with intellectual and developmental disabilities in Vermont. You will help plan, access, and coordinate medical, social, educational, and other supports to promote independence and community inclusion.

The role requires completing state training, coordinating assessments, and leading person-centered planning meetings with the individual and their team to ensure planning and services meet goals and preferences.

Qualifications

• Knowledge of Vermont public service system for developmental disabilities and brain injury services.
• Understanding of state statutes and program requirements.
• Ability to work collaboratively while meeting performance goals.
• Strong verbal and non-verbal communication with a broad stakeholder audience.
• Proficiency with technology tools in daily duties.

Responsibilities

• Complete core competency training required by the State.
• Perform evaluations and assessments to meet individuals' needs.
• Assist individuals in understanding goals and activities of person-centered planning.
• Facilitate and schedule person-centered planning and assessments.
• Complete Needs Assessment Annual Review Protocol with the individual and team.
• Assist individuals in exploring preferences, strengths, goals, and needs for planning.
• Facilitate the completion of budgeting and service planning documents.
• Gather and organize assessments and documentation for planning processes.
• Help individuals invite support team members to participate in planning meetings.
• Develop agendas and follow-up notes for planning meetings and distribute to the team.
• Support the individual to lead the planning process and implement plans.
• Identify modifications to rights and freedoms as needed.
• Review and update person-centered service plans and documents with the individual.
• Monitor plan implementation and satisfaction through visits and guardian/family input.
• Respond to emergencies/incidents to protect health and welfare.
• Ensure services align with the person-centered service plan and needs.
