Job summary

Hutchinson Regional Healthcare System is seeking a Case Manager to deliver community-based mental health services for children and adults with significant challenges. The role focuses on normalization of functioning while keeping individuals in the least restrictive home and community setting.

The position requires case management training (CPST, KHS), background checks, and a valid driver’s license. Weekend, part-time hours are included with benefits listed above.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field preferred.
• Associate’s degree with 2+ years’ experience teaching clients about self-care and daily living activities.
• Total combined schooling and experience equal 4+ years.

Responsibilities

• Provide skill training on target goals with measurable outcomes as determined by medical necessity.
• Deliver intensive, community-based, interactive mental health services to children and adults.
• Develop and maintain a client-centered Treatment Plan with family and treatment team.
• Assist clients in understanding symptoms and possible solutions.
• Coordinate with natural community supports and other service providers.
