Job summary

Hutchinson Regional Medical Center, Inc. is seeking a Case Manager - Carrefour for weekend coverage in Horizons Reno County.

This part-time role focuses on delivering interactive mental health services to adults and children with significant challenges, in home and community settings. You will develop and implement client-centered treatment plans, coordinate care with families and providers, and promote recovery in a least-restrictive environment.

Qualifications

• Required education as listed; relevant experience preferred in mental health or related services.
• Ability to develop and monitor client-centered treatment plans with families.
• Familiarity with community mental health services and supports.

Responsibilities

• Provide skill training on target goals with measurable outcomes determined by medical necessity.
• Deliver intensive, community-based mental health services to children and adults using a strengths-based approach.
• Develop and maintain client-centered Treatment Plans with family and treatment team.
• Help clients understand symptoms and possible solutions; support natural community supports.
• Coordinate mental/physical healthcare services and facilitate communication among stakeholders.
• Adhere to HIPAA and maintain professional relationships with clients, providers, and partners.
• Perform other duties as assigned.

Essential Responsibilities:

• Provide skill training on target goals and objectives with measurable outcomes, with measurements as determined by medical necessity by the Qualified Mental Health Professional
• Deliver intensive, community-based, interactive mental health services to children and adults utilizing a strengths-based, skill-building philosophy
• Participate in the development and maintenance of a client-centered Treatment Plan along with the family and treatment team
• Assist the youth, or adult, develop a basic understanding of their symptoms and possible solutions
• Assist the adult, or youth, and family to enlist natural community supports in developing solutions
• Assist in coordination of mental/physical healthcare services and facilitate communication between the client, service providers, family and community partners
• Act in accordance with the established mission, vision, and values.
• Abide by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
• Maintain effective communication and professional working relationships with patients/clients and their representatives, team members, contractors, physicians, peers, outside agencies, and the public.

Minimum Qualifications:

Required Education and Experience

One of the following is required:

• Bachelor’s degree, preferably in Social Work, Psychology, or some related field
• Associate’s degree with an additional 2 or more years’ experience working with educating and teaching clients about self-care, improving daily living activities
• Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years

Required License/Certifications/Registrations

• CPST Case Management training, KHS training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
• Completion of Adult Case Management training within 60 days of employment.
• Valid driver’s license, insurance, and a dependable vehicle if position requires driving.

Preferred Qualifications:

Preferred Education and Experience

Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics).

Three to five years’ professional experience in mental health services, crisis response, or a related field.

Familiarity with community mental health services.

Strong organizational skills

Intermediate proficiency with Microsoft Office

Physical Requirements:

• Light Work: Occasionally exerting up to 25 lbs - frequently exerting up to 10 lbs. 11-25% of the day may be standing or walking.
