Job summary

Hutchinson Regional Healthcare System seeks a Case Manager to deliver community-based mental health services for adults and children with significant challenges. The role focuses on creating a client-centered Treatment Plan and coordinating care to keep individuals in their home and community.

Responsibilities cover skill-building, collaboration with families and providers, and adherence to PHI/privacy rules, while supporting overall functioning and least restrictive environments.

Qualifications

• Bachelor’s degree, preferably in Social Work, Psychology, or some related field
• Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities
• Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years

Responsibilities

• Provide skill training on targeted goals with measurable outcomes.
• Deliver intensive, community-based mental health services to children and adults.
• Develop and maintain a client-centered Treatment Plan with family and treatment team.
• Assist clients in understanding symptoms and possible solutions.
• Help families enlist natural community supports for solutions.
• Coordinate mental/physical healthcare services and facilitate communication among stakeholders.
• Act in accordance with the mission, vision and values.
• Abide by PHI privacy regulations and protect health information.
• Maintain professional relationships with patients, families, providers and partners.

Minimum Qualifications

Required Education and Experience

• Bachelor’s degree, preferably in Social Work, Psychology, or some related field
• Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities
• Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years

Required License/Certifications/Registrations

• CPST Case Management training, KHS training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
• Completion of Adult Case Management training within 60 days of employment.
• Valid driver’s license, insurance, and a dependable vehicle if position requires driving.

Preferred Qualifications

Preferred Education and Experience

• Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics).
• Three to five years’ professional experience in mental health services, crisis response, or a related field.

Preferred License/Certifications/Registrations

Knowledge, Skills, and Abilities

• Familiarity with community mental health services.
• Strong organizational skills
• Intermediate proficiency with Microsoft Office

Physical Requirements
