Qualifications

• RN license in Virginia or compact state, or MSW from an accredited program required.
• Three years or more acute care experience required.
• Must provide record of licensures, certifications, training, and education at time of hire.
• Continuous recertification and maintenance of licensures, certifications, training required.
• BSN preferred with transcripts provided at time of hire.

Responsibilities

• Educates and empowers individual/caregiver toward self-care and independence.
• Collaboratively conducts assessment, planning, facilitation, care coordination, evaluation and advocacy.
• Assesses plan of care and monitors utilization of resources and progress.
• Interfaces with managed care organizations, external reviewers, and other payers to ensure CMS standards.
• Facilitates options and services to meet patient health care needs.
• Performs all other duties as assigned.
