The University of Virginia invites applications for an RN Case Manager in the Department of Pediatrics to support Blue Ridge Care Connection for Children in Charlottesville, VA. The role focuses on population management and relationship-based care within a multidisciplinary team to improve patient outcomes across transitions of care.

The successful candidate will have an accredited nursing degree, Virginia RN license, BLS, and at least 1 year of relevant experience; BSN and case management

Qualifications

• Graduate of an accredited nursing program.
• 1 year of relevant experience.
• Licensed to practice as a Registered Nurse in Virginia; BLS certification.
• BSN and national case management certification preferred.

Responsabilités

• Assess patients' physical, functional, social, and financial needs.
• Develop and implement an individualized plan of care.
• Coordinate services and refer to community resources.
• Communicate with primary care physicians and care team about patient status.
• Maintain accurate patient records and confidentiality.
• Support population-based care and transitions within the care continuum.
