Qualifications

• Bachelor’s degree in social service or related field.
• Must have a valid driver’s license.
• Knowledge of community resources preferred.
• Ability to learn, navigate, and work daily in the EHR system effectively.
• Strong written and verbal communication skills and good time management.

Responsibilities

• Conduct interviews to collect information for psychosocial assessments.
• Develop service/treatment plans with the treatment team.
• Provide crisis intervention and assist with safety planning.
• Schedule reviews of treatment plans and monitor progress.
• Prepare case records, reports and related administrative tasks.
• Engage with consumers and families to access supports and services.
