Roadrunner Health Services in Lovington, NM seeks a supervisory healthcare case manager to evaluate patients' medical and mental health needs, develop and monitor care plans, and coordinate referrals and discharge planning.

The role collaborates with in-house staff, hospitals, and community partners to ensure continuity of care, maintains confidential records, reports to leadership, and trains staff on mental health and safety protocols.

Qualifications

• High school diploma or equivalent.
• 1 year experience in medical case management, correctional healthcare, or similar high-security environments.
• BLS/First Aid & CPR Certification.

Responsabilités

• Evaluate patients' medical and mental health needs and develop individualized care plans.
• Coordinate with in-house medical staff, hospitals, and providers for continuity of care.
• Advocate for access to services and educate patients and staff on conditions and treatments.
• Maintain confidential medical records and prepare reports for leadership and stakeholders.
• Deliver training on mental health, safe release, and vulnerabilities; collaborate with community partners.
• Perform other related duties as required.
