Job summary

Pinnacle Services, Inc. in Minneapolis, MN, seeks a Case Manager Captain to lead a 6-9 member team within our case management program, serving individuals with developmental disabilities and behavioral health needs. This role requires supervisory experience and a Social Work license or eligibility.

You will assess needs, coordinate resources, monitor services, and participate in interdisciplinary teams, with opportunities for training and professional growth; hybrid schedule after training.

Qualifications

• 1+ year waivered case management experience (DD, CADI, AC/EW).
• Previous supervisory experience leading a team of 5 or greater.
• Valid driver’s license with acceptable driving record and vehicle with insurance.

Responsibilities

• Assessing areas of the persons served life where assistance is needed.
• Coordinating resources for the persons served being supported.
• Monitoring service delivery with providers to meet needs.
• Service plan development.
• Attend regular meetings with persons served and interdisciplinary teams.
• Lead a team ranging from 6-9 Case Managers.
• Provide consultations when needed.
• Assist with training subordinates.

Case Manager Team Leads have much responsibility while managing their caseloads and their team, therefore someone who is looking for ample opportunity to consult, collaborate, and lead others would succeed in this role.

Job Responsibilities and Duties

Case Manager Captains have the opportunity to work with persons served that may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or are aging.

• Assessing the areas of the persons served life where assistance is needed.
• Offering and coordinating resources for the persons served being supported.
• Monitoring service delivery and working with providers to ensure the person's served needs are met.
• Service plan development.
• Attend regular meetings with persons served and their interdisciplinary teams.
• Lead a team ranging from 6-9 Case Managers.
• Assist with answering questions.
• Provide consultations when needed.
• Assist with training subordinates.

Requirements

• 1+ year Waivered Case Management experience (DD, CADI, AC/EW).
• Previous Supervisory Experience leading a team of 5 or greater.
• Applicants must have a valid driver’s license and acceptable driving record as determined by Pinnacle Services policy. Applicants must have a vehicle that is covered with current insurance. Applicants must successfully clear local and/or stated background checks.

Preferred Qualifications

• LSW or Higher Form of BOSW Licensure.
• One year of experience in education or treatment of people with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities.
