Pinnacle Services is seeking a Case Manager Captain in Hennepin County to lead a 6-9 member Case Manager team, assess client needs, coordinate resources, and develop service plans. The role emphasizes supervision, collaboration, and consultative guidance within a fast-growing agency.

The position requires a bachelor’s in social work or licensure, and includes a hybrid work schedule, with opportunities for leadership and professional growth.

Qualifications

• 1+ year Waivered Case Management experience (DD, CADI, AC/EW).
• Previous Supervisory Experience leading a team of 5 or greater.
• Applicants must have a valid driver’s license and acceptable driving record as determined by Pinnacle Services policy.

Responsibilities

• Lead a team ranging from 6-9 Case Managers.
• Assess areas of the persons served life where assistance is needed.
• Offer and coordinate resources for the persons served being supported.
• Monitor service delivery and ensure the persons served needs are met.
• Develop service plans.
• Attend regular meetings with persons served and their interdisciplinary teams.
• Provide consultations when needed.
• Assist with training subordinates.
