Job summary

Helping Hands is seeking a dedicated Case Manager for our reentry program in Portland, OR. You’ll build supportive relationships, help participants access resources, and guide them toward long-term housing and stability.

This full-time role offers competitive benefits including medical, dental, vision, PTO, holidays, and a 401(k); occasional early mornings or evenings may be required.

Responsibilities

  • Build rapport with participants and learn about their needs through conversations and records review.
  • Meet monthly with participants to develop IRPs, set goals, review progress, and plan next steps.
  • Help participants understand and meet program requirements.
  • Assist participants in accessing housing, healthcare, SNAP, DMV, employment supports, and other services.
  • Coordinate with healthcare providers, counselors, DHS workers, sponsors, and parole officers.

Helping Hands provides HOPE and CARE through personal, trauma-informed programs to individuals experiencing homelessness. Our programs are Trauma-Informed, Data-Driven, and Person-Centered. We take the time to learn each individual's story and help connect them to the appropriate local resources and services to meet their unique needs. We do not duplicate available services; instead, we build strong collaborative partnerships with local programs and foster connections between participants and our community partners.
About the Role

As a Case Manager in our reentry program, you’ll build supportive relationships with participants, help them navigate resources, and work alongside them as they set and pursue goals. You’ll support program participation and help people take practical steps toward long-term, stable housing.

$22.71 per hour. This full-time position is eligible for medical, dental, and vision coverage, paid time off (PTO), Holidays, and 401(k). May require occasional early mornings, evenings, or weekends as needed.

What You’ll Do
  • Build rapport with participants and learn about their needs through conversations, records review, and consultation.
  • Meet with assigned participants monthly to develop and update Individual Reentry Plans (IRPs), set goals, review progress, and identify next steps.
  • Support participants in understanding and meeting program requirements.
  • Help participants access community resources and services, which may include housing, healthcare, SNAP, DMV appointments, employment supports, and other services.
  • Coordinate with professionals and community partners involved in participants’ success, such as healthcare providers, counselors, legal supports, DHS workers, sponsors, family members, and parole officers.
  • Respond to crises using trauma-informed de-escalation approaches and help connect participants with appropriate, timely resources.
  • Develop and maintain a network of community resources, including medical and mental health services, DUI classes, social supports, financial coaching, and legal assistance.
  • Build relationships with landlords, property managers, housing authorities, and affordable-housing partners; stay informed about housing opportunities and relevant requirements.
  • Provide practical education and resources to help participants secure and maintain permanent housing, including coordinating rental assistance applications and payments.
  • Maintain accurate documentation and protect participant confidentiality.
What You’ll Bring
  • High school diploma or equivalent.
  • At least one year of case management or social services experience.
  • Strong interpersonal, verbal, and written communication skills.
  • Careful documentation and basic use of technology skills, including email, Microsoft Word and Excel, and data entry.
  • A respectful, person-centered and trauma-informed approach and the ability to work collaboratively with participants and community partners.
Especially Valuable
  • Direct or lived experience related to homelessness, addiction, and/or working with low-income populations.
  • Understanding of issues that may affect participants, including homelessness, chemical dependency, domestic violence, and sexual assault.
Work Environment & Physical Requirements

Work is primarily indoors in an office setting, with occasional moderate noise and exposure to scents. The role frequently involves sitting or standing at a computer and using standard office equipment, with occasional movement throughout the facility for meetings and work needs.
