Westhab is seeking a Case Manager in the Bronx, NY to assess clients’ needs, plan service actions, link clients to housing resources and monitor progress in the program.

You will conduct intake, develop service plans, meet weekly with clients, coordinate with housing staff and partners, and advocate for entitlements. A bachelor’s degree and 1 year of case management experience are preferred; bilingual ability is a plus.

Qualifications

  • Bachelor’s degree in Social Work or Human Services preferred.
  • At least 1 year of case management experience with homeless, mentally ill, substance abuse and/or low income populations.
  • Bilingual a plus; strong relationship-building skills.
  • Strong organizational and time management skills; ability to multi-task.

Responsibilities

  • Conduct intake and assessment to orient client and develop an individualized service plan.
  • Meet weekly with client to support achievement of goals.
  • Coordinate services with Housing Specialist and external partners.
  • Advocate for clients around entitlements and services.
  • Provide crisis intervention and supportive counseling.
  • Document case management activities per guidelines.
  • Complete housing applications for clients as needed.
  • Achieve monthly placement quotas as assigned.
  • Facilitate workshops and escort clients as applicable.
  • Attend staff meetings and trainings; may work evenings/weekends.

SUMMARY:

The Case Manager assesses clients’ needs, plans a course of action, links client to services and monitors progress of clients and residents in their program. The Case Manager ensures that clients receive appropriate information and referrals to obtain and maintain housing.

DUTIES & RESPONSIBILITIES:

  • Document case management activities in accordance with agency and funding source guidelines and procedures.

  • Achieve monthly placement quota, as assigned.
  • Facilitate workshops, groups and escort clients as applicable.
  • Attend staff meetings and trainings to enhance skills that are related to job function.
  • Must be able to work flexible, alternate schedules which may include evening and weekend shifts.
  • Perform other related duties.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:

Bachelor Degree in Social Work or Human Services Field preferred. At least 1 year of case management experience working with the homeless, mentally ill, substance abuse and/or low income populations. Bilingual a plus. Strong relationship building skills required. Ability to work independently but know when to raise issues to supervisor.
