Felton Institute is seeking a Case Manager for a temporary Bridge Housing site in Santa Rosa. The role supports individuals with serious behavioral health conditions experiencing homelessness, coordinating care with a multidisciplinary team and connecting clients to housing and health services.

The position requires at least 2 years of relevant case management experience, a degree in a clinical field, and a commitment to strengths-based, harm-reduction approaches.

Competenze

• 2+ years of case management, substance use disorder and mental health treatment experience for TAY and adults (including co-occurring disorders)
• Bachelor's degree or higher in Social Work, Counseling, Psychology, or related field or currently enrolled in undergraduate degree program required
• Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills
• Knowledge of de-escalation, mitigation and pre-escalation techniques
• Willingness to work from a strength-based, harm reduction, client-centered model with evidence-based practices
• Food handler permit to serve food
• Provide support for clinical and administrative staff in a multi-disciplinary setting
• Other duties may be assigned to meet program goals

Mansioni

• Provide care and case management, mental health and substance use recovery and wellness services within a multi-disciplinary team
• Utilize evidence-informed interventions to address guests' needs and goals
• Collaborate with community partners acting as current access and intervention points
• Meet documentation and deadlines; comply with Medi-Cal requirements where applicable
• Perform administrative tasks within timelines (chart completion, meeting attendance, etc.)
• Connect guests with housing assistance, transportation vouchers, emergency food gift cards, and grooming/hygiene products
• Assist guests in applying for public benefits
• Link guests to primary care and other health services
• Help reduce hospitalization, homelessness and incarceration via re-integration into the community
• Develop and provide gender and culturally responsive curricula
• Food prep tasks as needed
• Develop trainings addressing domestic violence, trauma, substance use, housing displacement
