Job summary

The W Group in New York is seeking a compassionate Case Manager to coordinate services for residents, ensuring excellence in resident satisfaction. You will work with a positive and supportive team of healthcare professionals to deliver coordinated care.

Qualifications

• Prior experience in Long Term Care is a PLUS or 1 year of experience as a Case Manager required.
• Previous Compuware experience is a plus.
• Excellent phone skills and ability to work with families/other care providers.
• Strong organizational skills, passion and compassion for the population.
• Ability to work cooperatively with multidisciplinary team.

• To communicate with the resident’s care providers.
• Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals.
