Job summary

Centers for Independence in Milwaukee, WI seeks a dedicated Case Manager to support individuals with severe mental illness and/or substance use disorders within an integrated treatment team.

You will develop client-centered treatment plans, coordinate community resources, monitor psychiatric medications, provide crisis support, and help clients secure housing and income resources, while meeting program metrics and documentation requirements.

Qualifications

• Bachelor's degree in behavioral science or related field.
• Minimum 1000 hours of post-degree clinical experience with individuals with severe and persistent mental illness.
• Valid Wisconsin driver's license and access to a personal vehicle.

Responsibilities

• Builds therapeutic rapport to identify treatment needs and create client-centered plans.
• Provides psychosocial rehabilitation, counseling, and crisis support.
• Monitors psychiatric medications and collaborates with physicians.
• Coordinates with community resources and housing/income supports.
• Maintains documentation and billing in accordance with program policies.
