Job summary

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• Minimum of 1 year working with adults with severe and persistent mental illness.
• Valid Wisconsin driver’s license and access to reliable transportation.

Responsibilities

• Develops treatment relationships and formulates treatment plans for clients.
• Provides psychosocial rehabilitation, symptom monitoring, and supportive counseling.
• Monitors psychiatric medication regimes and ensures adherence.
• Coordinates access to community resources, housing, and outside providers.
• Performs timely documentation and maintains client case files.
• Participates in team case management and discharge planning.
