Job summary

Youth Opportunity in Nashville, TN seeks a Case Manager to coordinate assessment services, documentation, and transition planning at Harpeth Primary Assessment Center. You will serve as a liaison among youth, families, the Tennessee Department of Children's Services, the treatment team, and community partners.

The ideal candidate is organized, communicates effectively, and can work independently under clinical supervision.

Qualifications

• Bachelor’s degree in counseling, social work, psychology, rehabilitation, special education, or related field.
• Strong verbal and written communication skills.
• Ability to maintain accurate documentation and meet deadlines.

Responsibilities

• Provide case management services and maintain ongoing communication with youth, families, agency representatives, and treatment team.
• Coordinate mental health and substance use screenings and assessments as required.
• Prepare and review treatment plans, transition plans, and discharge summaries.
• Maintain accurate case records per organizational and contractual requirements.
• Serve as liaison among the program, the Tennessee Department of Children’s Services, outside agencies, and families.
• Support youth and families for program visits, conferences, and aftercare responsibilities.
