Qualifications

  • Experience with multi-system navigation in public/human services.

Responsibilities

Job description

Responsibilities
  • Conduct comprehensive evaluations of referred members' needs and eligibility
  • Evaluate benefit plans and internal and external programs/services to recommend case-resolution approaches
  • Identify high-risk factors and service needs affecting member outcomes and care planning
  • Coordinate and implement assigned care-plan activities and monitor progress
  • Consult with case managers, supervisors, Medical Directors, and health/behavioral health programs to overcome barriers
  • Present cases at case conferences for multidisciplinary review
  • Collaborate with members' Child and Family Teams
  • Identify and escalation quality-of-care issues
  • Negotiate appropriate options and services to meet members' benefits and healthcare needs
  • Use motivational interviewing and influencing skills to engage members and promote behavior changes
  • Provide coaching, information, and support for independent medical and healthy-lifestyle choices
  • Help members participate knowledgeably in healthcare decision-making with providers
  • Serve as a single point of contact and address immediate gaps in care and access
  • Apply case management and quality management processes in compliance with regulatory, accreditation, and company requirements
  • Travel to member homes and other member locations within Northeast Ohio
Requirements
  • 2+ years of experience in behavioral health, social services, or human services
  • 2+ years of experience with personal computers, keyboard and multi-system navigation, and MS Office Suite applications (Outlook, Word, Excel, SharePoint)
  • 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services or behavioral health care, providing community-based services to children and youth and their family/caregivers
  • 2+ years of experience in family systems, community systems and resources, case management, child and family counseling/therapy, child protection, or child development
  • 2+ years of experience with Ohio delivery systems, including local community networks and resources
  • Willing and able to travel within the assigned region 50% of the time or more
  • Some travel to the New Albany office may be required for trainings/meetings
  • Reliable transportation required
  • Willing and able to work beyond Monday-Friday, 8am-5pm, as needed
  • Bachelor's degree or non-licensed master's-level clinician required, in behavioral health, human services, health services, or public health
  • Candidates must reside in Ohio; candidates in the Northeast region or adjacent counties are eligible, with preference for Seneca, Lorain, Medina, Wyandot, or Crawford County
  • Case management and discharge planning experience preferred
  • Managed Care experience preferred
  • Medicaid experience preferred
